GCM – Case Profile Submission

The Central Missouri Affiliate of the Midwest Geriatric Care Management Chapter is compiling case profiles from fellow GCM’s. These profiles will be used for presentation purposes.

It is often difficult to explain what a GCM does. This collection of case profiles will provide GCM’s with examples of cases we have worked with. It is very important when submitting these profiles that you do notuse any identifying information. It is important to include age, gender and an overview of why GCM services were needed and the outcome. See example case profiles.

Once we have finalized the collection we will be happy to email the file to all participating GCM’s.
